Ascension Personalized Care, underwritten by US Health and Life Insurance Company, offer individual Marketplace health plans in Michigan, Indiana, Kansas, and Texas beginning Jan. 1, 2024.

Ascension Personalized Care is a new ACA health plan built by Ascension to deliver the coverage and benefits that matter most to achieve and maintain personal health goals.

Plans will be offered in 3 states, 4 markets:

Indiana (Indianapolis area counties): Boone, Carroll, Clinton, Fountain, Hamilton, Hancock, Hendricks, Henry, Howard, Johnson, Madison, Marion, Montgomery, Putnam, Randolph, Shelby, Tipton, Warren, and White.
Indiana (Evansville area counties): Davies, Gibson, Jennings, Knox, Martin, Perry, Posey, Spencer, Vanderburgh, and Warrick
Kansas (greater Wichita area counties): Butler, Cowley, Harper, Harvey, Kingman, Sedgwick, and Sumner
Michigan (greater Detroit area): Oakland, Macomb, and Wayne
